I am posting my sample. Hope it gets you started.
You will need to tweak it to add the fields that you need but a sample is always good to get started.

Usage notes:
- paste your data in columns A-G
- The excel sheet will convert the values into columns H-O
- Copy the formulas from row-1 columns H-O for all rows of your data
- Now copy the columns H-O for all rows of your data
- Paste the values into a text editor and save the resulting file to your disk.
- That should be it
